Titles are already distributed with the pledges so for the launch I don't believe they will put in much more titles.
Yeah you are right, that titles can be called achievements, because those are achieved some way. However, I guess a lot of people perceives achievements like in WoW. I do not personally have anything against that kind of system either, but DS could create some really meaningful achievements without things like Jetah mentioned, or killing critters and other lame stuff like that. Actully I would like to see achievements more like trophies which are actual cosmetic/vanity items what you can place in your house/estate as decoration.
Back to titles then... Titles could be combined with knowledge system which will be there anyway so it does not cause that much extra work. When you unlock something rare, hard to accomplish and meaningful, then maybe you could get a title also as reward. Or maybe when you have unclock X% of the whole knowledge tree it could give you something nice, like Tuoni the "knowledgeable", Tuoni the "wise" or something like that.
People love titles, okay not all of them, but a good amount of people does. They want to sound cool, they want to show others their status and they like a little goals like these. So Why not? If this can be implemented with small effort, I do not see any downsides.
